Blocked soffit vents
The chain
Cold air in at the soffit → up under the roof deck → out at the ridge. That continuous flush is what keeps the deck the same temperature as the outside air. A cold deck doesn't melt snow. No meltwater, no dam.
Break the chain at the soffit and everything downstream follows: attic heat warms the deck, snow on the upper roof melts, water runs down to the eaves — which hang over open air and stay freezing — and refreezes there. The ridge of ice grows, water pools behind it, and it finds its way under the shingles. Your gutter is just where you notice it.

The top one is worth dwelling on because it's usually well-intentioned. Someone adds insulation to save on heat, pushes it out to the very edge of the attic floor to be thorough, and in doing so plugs every soffit vent in the house. More insulation, worse ice dams. Without baffles, that's what happens by default.
How to check — two minutes
From the attic (the real test): pick a sunny day, go up, and look along the eaves where the roof meets the attic floor. You should see daylight at the vents and an open channel above the insulation. If insulation is jammed against the sheathing and there's no light, they're blocked.
From the ground: look up at the soffit panels under your eaves. Are the vent slots or perforations painted over? Full of dirt or nests? A soffit with no visible venting at all?
The fix
Baffles — rigid channels stapled between rafters at the eaves that hold insulation back and preserve the air path. Roughly $2–$5 each plus labor; a typical attic runs $300–$800 installed. This is the fix, and it's cheap relative to what it prevents.
Clear the vents — pull insulation back, clean debris, scrape paint out of the slots.
Add venting if there genuinely isn't any — common on pre-war NJ homes.
Balance intake and exhaust. Ridge vents without adequate soffit intake don't work — the ridge has nothing to pull. A lot of homes have a ridge vent that's essentially decorative because the soffits are plugged.

Soffit vents blocked by insulation
Of all the ways a soffit vent gets choked, insulation is the one that does the most damage and shows the least on the outside. Attic insulation isn't fixed in place — batts sag, and blown-in cellulose or fiberglass migrates. Over the years it slides down the slope of the attic floor toward the eaves, and the eaves are exactly where the soffit intake vents pull their air. The insulation drifts right into that intake and buries it. The homeowner who added an extra layer to cut heating bills usually makes it worse, because thicker insulation at the edge means a tighter plug.
Once the intake is buried, the ventilation chain has no starting point. Cold outside air can't enter at the eaves, so nothing flushes the underside of the roof deck. Attic heat sits against the sheathing, warms it, and the roof-deck temperature climbs above freezing. Snow on the upper roof melts, the meltwater runs down to the cold overhanging eaves, and it refreezes there into an ice dam — the same failure mode as a painted-over or debris-clogged vent, just hidden inside the insulation where you'd never spot it from the street. More insulation with no intake air is a colder attic floor and a warmer roof deck, which is the worst combination for ice dams.
Diagnose it from the attic. Pick a sunny day and look down the line where the roof deck meets the attic floor. You want to see daylight at each soffit vent and a clear gap above the insulation. If the insulation is jammed up against the sheathing at the edge and the light is gone, the intake is blocked — feel for airflow on a windy day, or note if the eave insulation looks compressed and shoved outward.
The fix is baffles (rafter vents). These are rigid channels stapled to the underside of the roof sheathing between the rafters at the eaves. They hold the insulation back off the deck and preserve an open air path from the soffit vent up past the insulation to the deck. Pull the insulation back, staple in a baffle at every rafter bay over an intake vent, then let the insulation settle against the baffle instead of the sheathing. Roughly $2–$5 per baffle plus labor, about $300–$800 for a typical attic — cheap against the roof and interior damage an ice dam causes. Baffles are also what stops the insulation from drifting back into the vents next winter, so it's a fix that stays fixed.
